Order Form

If you would like to place an order off of our web site, please fill out the form below. You will be contacted to verify the information and an invoice for the order will be e-mailed that includes the order total with tax and shipping charges, an estimated processing and delivery time along with our order terms and policies. Your order will not be processed until your payment in full is received. A 4-6 week standard shipping and processing time apply.

If you are looking for a custom design, please fill out the form below. Once your custom design request is received we will contact you to get more information so we can better understand your vision. You will then receive a confirmation e-mail and a bill for the non-refundable deposit fee of $500 that will be credited towards your final bill. Upon receipt of payment you will be given three different design options (based upon your ideas and
suggestions). Once a final product is agreed upon you will be billed for the total amount due. A maximum of three rounds of revisions will be permitted without accruing additional charges. Your order is not processed until your
payment in full is received. Pleases allow 10 weeks from conception to completion for your order. *We will accommodate rush projects for an additional fee.
